If available, bring any current X-rays, CT scans, MRIs, or films along with their reports. Previous operative reports would also be helpful. Additionally, please bring your insurance card and any required referral forms.

**Due to federal regulations, we are unable to see patients who are enrolled in Medicare or Medicaid, even on self-pay basis.
Medicare patients can still choose cosmetic prosthetics; however, they are not covered by Medical One insurance.
